Primary Function:

 

To provide individualized support for students with IEPs returning to their home schools from IEP private school or residential placements according to IEPs and Transitional Plans of Support.  The Transitional Specialist will implement the IEP/Plan of Support designed to assist the student in transitioning successfully back into a public-school setting by providing support in academic, behavioral, social, and emotional areas as outlined in the IEP/Plan of Support.

 

Qualifications:

 

  1. Holds at least an Associate degree; Bachelor’s degree preferred
  2. Demonstrates basic reading, writing and computation skills.
  3. Has previous experience working with children in a public-school special education setting.
  4. Possesses knowledge and understanding of effective behavioral strategies.
  5. Possesses strong interpersonal skills and ability to work collaboratively in a team environment.
  6. Ability to maintain confidentiality and professionalism in all interactions.
  7. Physical ability to assist students according to specific needs (i.e., mobility, personal care, behavior, etc.).
  8. Ability to learn new skills and strategies that may be needed to address specific student needs.
  9. Has good physical and emotional health

 

Reports To:  Designated Supervisor in School Division and PREP Executive Director

 

Performance Responsibilities:

 

  1. Provide individualized support for the student according to IEP/Plan of Support.
    • Assist with implementation of IEP goals/objectives and ensuring accommodations/modifications are in place.
    • Assist in implementation of behavior, social, and emotional interventions.
    • Assist with classwork, assignments, and activities.
    • Assist in managing and addressing challenging behaviors through positive behavior support strategies, implementation of Behavior Intervention Plan (BIP), and providing direct instruction of social skills and self-regulation skills.
    • Work collaboratively with classroom and special education teachers and other specialists to reinforce instruction and adapt instructional materials and strategies.
    • Provide support in accessing assistive technology devices and adaptive equipment to enhance student’s access to the curriculum and promote independence.
    • Assist student as needed with personal care tasks, mobility, and physical accommodations.
    • Collaborate and communicate effectively and regularly with school staff.
    • Collect data on student progress and behavior.  Keep accurate records of student performance, behavior incidents and interventions implemented. 
    • Follow individual student plans and school protocols for crisis intervention to ensure the safety and well-being of the student and others.

 

  1. Assist the student in developing functional independence in the school setting.

 

  1. Perform other duties as assigned by PREP Executive Director.

 

Terms of Employment:

 

            200 Day Contract; Salary established from Support Staff Salary Scale
